New Patient Forms

In an effort to make your first visit to Dental Care East Hanover as convenient and efficient as possible, we offer a variety of ways for you to submit your information that will help us get to know you and your oral health condition before you visit our office. To notify our staff and complete the necessary paperwork, please select one of the following options:

Complete the secure New Patient Form online, which can be accessed via computer/laptop, smartphone, and tablet devices. Once electronically submitted, the form will be sent to our office staff. No need to print anything out!

Download the New Patient Form [PDF] from the website and print it out. After you complete filling out the form, just remember to bring it with you to the office when you arrive for your appointment. (Note: Adobe Acrobat Reader may be needed to download the form.)

Request a New Patient Form at the front desk when you arrive to the office. If you decide to go with this option, we request that you arrive at least 15 minutes early to give yourself sufficient time to complete the form before your scheduled appointment.
