- No membership card is needed — your plan’s effective date is on file in our office
- NON-REFUNDABLE. If the patient decides to not use the plan, we will not issue any refunds or premiums
- The full annual fee is due at the time of enrollment
- If the plan lapses, a $50 reactivation fee will be charged
- Benefit coverage is subject to annual review and change
- Cannot be used in conjunction with another dental plan

- For services and injuries covered by workman’s compensation
- For any charges related to hospitals or hospitalizations
- For dental care costs covered under a medical, automobile, or any other claims
- For any procedure or treatment that lies in what the treating dentist/doctor believes is outside the realm of their capability
