It is our goal to ensure that your first visit is a stress-free success, so we give you plenty of time to meet your dentist before any treatment begins to address any questions and concerns that you might have.

We keep extra copies at the office, in case you forget to print out the forms or do not have access to a printer. Please just remember to arrive 5-10 minutes earlier than your scheduled appointment to fill them out.
